AdaptCare

AdaptCare is an application designed to dramatically increase patient healthcare outcomes during an emergency. It displays a patient’s adaptive care plan from a hospital’s electronic medical records to a mobile device using a QR code. The QR code is printed on a wristband, sticker or similar item that remains with the patient giving emergency healthcare providers, such as EMTs, firefighters and other personnel who may not be credentialed at the hospital instant access to a patient’s particular needs to ensure continuity of care. AdaptCare can also alert providers that the patient may be non-verbal, deaf, blind, have sensory triggers or other special communication needs, even when a family member or guardian isn’t present.

This gives emergency healthcare providers, such as EMTs, firefighters and other personnel who may not be credentialed at the hospital, instant access to a patient’s particular needs to ensure continuity of care. AdaptCare brings real-time data and templatization through integration with Children’s Nebraska Epic (Epic is a registered trademark of Epic Systems Corporation) EMR for their Project Austin and PATCH programs. Deploying these successful programs on the AdaptCare platform allows the staff teams to increase their efficiency through automation, have tighter version control over plan documents, replace workflows involving printed documents and increase the program reach into the community.

Project Austin Emergency Medical Plan

Project Austin fosters continuity of care for children with medical complexity by collaborating with emergency medical services (EMS) and hospital emergency departments to build community awareness, provide education and devise an emergency plan for the child.

  • Encourages collaboration among caregivers, EMS personnel and emergency health care workers.
  • Provides an Emergency Information Form (EIF) that contains important medical information in an emergency.

PATCH Behavioral Adaptive Care Plan

PATCH (Patient Assistance Team at Children’s Hospital & Medical Center) helps improve a neurodivergent child’s experience by creating a personalized behavioral adaptive care plan to use when a child comes to the hospital. Whether it is a trip to the emergency room, a scheduled procedure, same-day surgery or an inpatient hospital admission, the adaptive care plan helps the care team understand a child’s unique needs and prepare for their visit.
